According to the Arizona Department of Public Safety, three persons from Colorado died in a multi-vehicle incident in Arizona on Friday, March 14.

At around 1:34 p.m., troopers responded to the incident on westbound Interstate 40 at milepost 294, near Holbrook. Five persons died in the incident, while two more were injured. According to authorities, one person was unhurt in the crash.

The crash killed the following people:

  • Jeet Vikram, 34, from Ozone Park, New York.
  • Sarah Broyel-Case, 50, from Fort Collins, Colorado.
  • Adam Mychajliw, 17, from Scottsdale, Arizona
  • Sophia Case, 17, from Fort Collins, Colorado.
  • Samuel Case, 51, from Fort Collins, Colorado. (Case died in the hospital on Tuesday)

According to authorities, the early investigation revealed that a semi-truck rear-ended a Cadillac Escalade, triggering a chain reaction involving a Jeep Wrangler and another semi-truck.

The incident occurred amid significant traffic congestion, which authorities blamed on detours from winter storm-related road closures in northern Arizona.

Police say the crash is still being investigated.
